Objective

The goal of the diabetes mobile is to provide diabetes education and support to individuals at risk of and who may be living with diabetes. We will do this by working with you, and your health care providers.

The NorWest Community Centres’ diabetes mobile health care team consists of:

• Nurse Practitioner - Our Nurse Practitioner will monitor your glucose levels, blood pressure, and cholesterol. They will work with you and your family physician to manage your diabetes.
• Foot Care Nurse - Our Foot Care Nurse provides preventative foot care for clients living with or at high risk of developing diabetes.
• Community Health Worker - Our Community Health Worker is available for health promotion and prevention programs and will assist clients in finding supports for health and social well-being.
